https://osu-ppy-sh.tvgratuite.org/b/189 - instead of Insane, theres a Original Insane diff inside the osz which is unsubmitted.

There are more examples than just that. This is from the retired beatmap error thread. Some of them are because of copyright issues. The older versions are the problems maps.
